MFP – Money Follows the Person

The MFP waiver assists individuals transitioning from long-term care facilities to home-based care. It offers various support services to help make this transition smooth and sustainable.

Eligibility Criteria

  • You must be transitioning from a long-term care facility, such as a nursing home or hospital, back into the community.
  • You must be eligible for Medicaid (MassHealth) and meet clinical criteria for home-based care.

TBI – Traumatic Brain Injury Waiver

The TBI waiver provides community-based services for individuals who have sustained traumatic brain injuries. These services are designed to offer personalized care and support, allowing participants to continue their recovery at home while maintaining independence.

Eligibility Criteria

  • You must be diagnosed with a traumatic brain injury.
  • You must require ongoing support and services to live safely at home or in a community setting.
  • You must meet Medicaid (MassHealth) eligibility requirements.

FEW – Frail Elder Waiver

The FEW program is intended for seniors who are eligible for nursing facility care but prefer to receive services at home. This waiver allows frail elders to live in their own homes or community settings while receiving essential support.

Eligibility Criteria

  • You must be over the age of 60 and require a level of care typically provided in a nursing facility.
  • You must prefer to receive care at home or in a community setting.
  • You must be eligible for Medicaid and meet clinical eligibility criteria.
  • Did you know? Under the FEW program, you can choose your own caregiver, including a family member or trusted friend, to provide your care and they’ll get paid.
