Group Adult Foster Care
Group Adult Foster Care (GAFC) is a Personal Care Assistance Program sponsored and paid for by MassHealth (Massachusetts Medicaid). This program keeps individuals safe at home and reduces the need for hospitalizations and admissions to nursing homes for more care. There is no out of pocket expense and MassHealth pays for all of the cost of the program.
Eligibility Criteria
- Must be 22 years or older
- Live in subsidized housing
- Have a primary care physician
- Require assistance with at least 2 ADL (Activity of Daily Living)
- Meets MassHealth income requirements
