Adult Foster Care (AFC) is a program designed to provide personalized care and support to adults who need assistance with daily living activities in a home-like setting. This service is ideal for individuals who prefer a more private and individualized care environment.

AFC Services Include:

• Personal Care Assistance: Support with daily activities such as bathing, dressing, grooming, and toileting.
• Medication Management: Help with medication administration to ensure correct and timely intake.
• Homemaker Services: Assistance with household tasks such as cleaning, laundry, and meal preparation.
• Companionship: Emotional support and companionship to enhance mental and emotional well-being.
• Respite Care: Temporary relief for primary caregivers, allowing them to take a break while ensuring continuous care for their loved ones.
• Health and Wellness Monitoring: Regular health assessments and coordination with healthcare providers to manage medical conditions.

Member Eligibility:

To be eligible for AFC, members must:

• Be 16 years or older.
• Require daily assistance with at least one activity of daily living (ADL).
• Be enrolled in a participating MassHealth program.
• Reside in a private home with a qualified caregiver.

Caregiver Eligibility:

Caregivers in AFC program must:

• Be at least 18 years old.
• Pass a background check.
• Complete required training and orientation.
• Be able to provide the necessary care and support for the member.

You Can Get Paid to Care for Your Loved One!

Through our AFC program, family members and close friends can become paid caregivers. This program allows you to provide care for your loved one in a familiar and loving environment while receiving financial compensation for your dedication and support.
