Group Adult Foster Care

Group Adult Foster Care (GAFC) is a Personal Care Assistance Program sponsored and paid for by MassHealth (Massachusetts Medicaid). This program keeps individuals safe at home and reduces the need for hospitalizations and admissions to nursing homes for more care. There is no out of pocket expense and MassHealth pays for all of the cost of the program.

Eligibility Criteria

Must be 22 years or older
Live in subsidized housing
Have a primary care physician
Require assistance with at least 2 ADL (Activity of Daily Living)
Meets MassHealth income requirements

Services Covered Under GAFC Program

GAFC Program Services Include:

Personal Care

Assistance with bathing, dressing, grooming, and toileting.

Homemaking

Help with housekeeping, laundry, and meal preparation.

Medication Reminders

Ensuring that medications are taken as prescribed.

Care Management

Ongoing care coordination and health monitoring.

Does GAFC provide nursing care?
